Please note:

  • Customers with medical conditions and/or expectant mothers beyond 28 weeks of pregnancy are requested to download the Medical form and Indemnity Form. Updated copies of both the forms would be required to be submitted at least 48 hours before the scheduled flight departure.

Unaccompanied Minor Form
